The Root Cause Administration now supports parent and child relationships with a new Sub Sections option. Only Corporate RCI users can change these root causes but all users will now see a root cause tree in their incident investigations.

Please contact us with any questions at 800-888-9596.

There is a new checkbox that appears on your incident forms entitled, "Prevent manual entry". When a Corporate RCI user checks this box on an incident form that particular incident will disappear from drop-down lists and will only show up on the Open/Closed Incident Investigations page. This function prevents users from manually creating these incident types. For example, some users use this function for their Audit incidents and Behavior Based Safety incidents.

We had a couple of requests for the date range on Incident Reporting screens to be set to this range or that range by default. While thats a great idea with over 20,000 accounts it becomes a challenge to handle all requests for this so we did the next best thing we let the users decide what their individual date range will be.

On the 'Recordable / First Aid Incidnet', 'Other Incident', and 'Incident Investigation' screens you will find a check box labeled 'Hold Dates'. This check box will hold the dates set in the 'Start Date' and 'End Date' selections and use those dates each time the page is visited. Unchecking the box will allow the date fields to use the default date range.

The Technical Explanation An organization’s appetite for risk is a numerical assignment of a quantified level of risk that a company is willing to accept.  This risk level, or appetite for risk, can be developed and assigned based on single activities, categories of activities, job types, or organizationally.  If an organization has not formally defined an appetite for risk, one will still exist, but will not be known and thus it will not be manageable.  For example, most companies allow employees to operate equipment and motor vehicles during the normal course of business, and this is an acceptable level of risk.  However, the majority of companies would not allow employees to operate motor vehicles if the employee had been recently convicted of driving under the influence.  This condition would push the level of risk beyond what the company is willing to accept, even if it hasn’t been clearly defined in a numerical manner.

The methodology used for defining the level of risk may be based on any number of Risk Assessment Techniques.  The key consideration is to be sure the risk assessment scale is calibrated for the organization and is kept consistent.  Under these conditions, any one risk can be evaluated (quantified) and compared (qualified) against any other risk.  The ability to compare the relative risk level of motor vehicle accidents to ergonomic related injuries, and to set the appropriate appetite for risk for each, is the hallmark of a well designed Risk Assessment Methodology.

As some people may have noticed we have instituted a new upload paradigm for the site. Now you should see a progress bar for uploaded files when you choose them. Many places you can also choose many files at once. We will be rolling out specific help screen shots for this soon but for right now please remember one thing 'SAVE Your Work'. The files are uploaded to the server as soon as you pick them but they are not saved with your incident/policy/etc until you hit the save button.

We were hoping this would be a little more transparent but we have had a few issues with the control in the live setting - we will be working those issues out over the next day or so. Feel free to call if you need help with this new feature.
